3. Interview consent forms
Crip Life™ uses Google Forms to collect consent from people who participate in interviews for publication on the website and related Crip Life™ platforms.
The interview consent form may collect information such as your name, email address and confirmation that you consent to being interviewed and to the resulting interview being edited, published and shared by Crip Life™. It may also record any specific permissions or preferences provided by you as part of the consent process.
Information submitted through the interview consent form is used to document and manage your consent and to support the publication and management of your interview.
We retain consent records for as long as reasonably necessary to demonstrate that appropriate permission was obtained and to manage the published content.
The form is hosted using Google Forms, meaning information submitted through it is processed using Google’s services.
Please do not provide sensitive or confidential personal information through the consent form unless it is specifically requested and necessary for the interview consent process.
